• Medical Billing: Claims submission, denial management, payment posting, accounts receivable follow-up, and revenue cycle optimization.
• Insurance Verification: Eligibility verification, benefits confirmation, and prior authorization support.
• Collections & Revenue Recovery: Accounts receivable management and structured recovery strategies for outstanding balances.

The specific scope, terms, and fees for any engaged services will be defined in a separate service agreement or statement of work between LL Billing and the client.

3. Use of Website

You agree to use our website only for lawful purposes and in accordance with these Terms. You agree not to:

• Use the website in any way that violates applicable federal, state, or local laws or regulations.
• Submit false, misleading, or inaccurate information through our forms.

4. Client Responsibilities

When engaging our services, clients are responsible for:

• Providing accurate, complete, and timely information necessary for credentialing, billing, or other services.
• Maintaining valid and current licenses, certifications, and insurance as required by applicable laws and regulations.
• Responding promptly to requests for information or documentation from LL Billing.
• Reviewing and approving submissions, applications, and claims before final processing when applicable.
• Complying with all applicable healthcare laws and regulations, including HIPAA.

5. No Guarantee of Results

While LL Billing strives to provide high-quality services and support, we do not guarantee specific outcomes, including but not limited to: approval of credentialing applications, acceptance into insurance panels, specific reimbursement amounts, or resolution of outstanding claims. Credentialing timelines and outcomes are subject to the policies and processes of individual payers and regulatory bodies, which are beyond our control.

6. Fees & Payment

Fees for our services are outlined in the individual service agreement between LL Billing and the client. Payment terms, billing cycles, and accepted payment methods will be specified in that agreement. LL Billing reserves the right to suspend or terminate services for non-payment in accordance with the terms of the applicable service agreement.

7. Confidentiality

LL Billing treats all client information as confidential. When our services involve access to Protected Health Information (PHI), we will execute a Business Associate Agreement (BAA) with the covered entity and comply with all applicable HIPAA requirements. We maintain appropriate administrative, technical, and physical safeguards to protect confidential information.

8. Intellectual Property

All content on this website — including text, graphics, logos, images, and software — is the property of LL Billing & Financial Services LLC or its content suppliers and is protected by United States and international copyright laws. You may not reproduce, distribute, modify, or create derivative works from any content on this website without our prior written consent.

9. Limitation of Liability

12. Governing Law & Dispute Resolution

These Terms of Service shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions. Any disputes arising under or in connection with these Terms shall be resolved through good-faith negotiation. If negotiation fails, disputes shall be submitted to binding arbitration in Miami-Dade County, Florida, in accordance with the rules of the American Arbitration Association.
